Health Clearances

State of Hawai‘i Department of Health (DOH) requires certain health requirements be met in order to attend a post-secondary institution. Please answer the following questions to make sure you understand the requirements.
These are the health clearance documents required:

Tuberculosis (TB) clearance within a year of the start of the semester

Tdap (Tetanus-diphtheria-acellular pertussis) 1 dose

MMR (Measles, Mumps, Rubella) 2 doses

Varicella (chickenpox) 2 doses

COVID-19 proof of full vaccination
You can submit your health clearance documents by:

Take pictures of TB, MMR, Tdap, Varicella documents and email them to Admissions (kapinfo@hawaii.edu)

Upload the COVID-19 vaccine card information to UH Lumisight (https://uh.campus.lumisight.com/home)

Drop off physical copies of TB, MMR, Tdap, Varicella documents to Admissions (‘Ilima Building, Room 102)
If you cannot submit health clearance documents or receive the immunizations / vaccines you will still be able to register for classes, but only online classes. If you cannot receive the immunizations / vaccines because of a medical condition or religious requirements, you can submit a Request for Exemption. See more information HERE.
